Eligibility and Requirements
- For most plans, sole proprietors and contractors must operate their business using a federal tax ID and be actively working.
- Dependent coverage may require verification of relationships – may include marriage certificates, birth certificates or other supporting documents.
- Enrollments finalized by the 15th of the month will be effective the following month. Any enrollment after the 11th will be effective the month after the following month.
- For example: an enrollment finalized on February 15 will become active March 1; an enrollment finalized February 18 will become active April 1.
